To apply for a driver license, you must visit a BMV location near you and present documents of identification and provide a signed Log of Supervised Driving.
The following are the requirements for applying for a new Indiana driver license:
- Must be 15 years or older.
- If under 18 applicant should provide parental consent and must complete the graduated driver licensing process.
- Must pass the vision, written, and driving tests.
- Applicants must provide acceptable proof of identification (see below)
- Applicants must provide proof of Indiana residency

Probationary Driver License
If you are 16 or 17 when you obtain your Indiana driver license, your license is considered probationary until you turn 18 years old. If you are 18 years of age or older when you obtain your Indiana driver license, your license is not probationary.
Required Documentation
New driver licenses and permits are issued by the state of Indiana at BMV license branches. All applicants for new driver licenses or permits must meet Indiana SecureID requirements by providing documents verifying their identity,
Be sure that you have acceptable documents before you visit a license branch. When you apply for a new driver license, permit, or identification card, you must present original versions or certified copies of the following documents:
- One document proving your identity; and
- One document proving your Social Security number; and
- One document proving your lawful status in the United States; and
- Two documents proving your Indiana residency.

Proof of Identity
The most common documents that you can use to prove your identity are:
- United States Birth Certificate (must be an original or certified copy)
- United States Passport, or a foreign passport with a VISA and I-94 form. Puerto Rican-born citizens must provide a birth certificate issued on or after July 1, 2010.
- If your current legal name is different from the name on your birth certificate or lawful status document, you must show legal proof of the name change (showing a link to all name changes).
*Note: If you do not have your original birth certificate in your possession, you need to contact your state or county Department of Health office of the state you were born and request an original or certified copy.
Proof your Social Security number when applying for a new driver license, permit, or identification card.
The most common documents that you can use to prove your Social Security number:
- Social Security card
- W-2 form
- SSA-1099 form,
- Non-SSA-1099 form
- Pay stub with both your name and Social Security number on it
Proof of lawful status in the United States
The most common documents that you can use to prove your lawful status in the United States are a United States:
- United States Birth Certificate (must be an original or certified copy)
- United States Passport
- A foreign passport with a VISA and I-94 form.
Note: Puerto Rican-born citizens must provide a birth certificate issued on or after July 1, 2010.
Proof of your Indiana Residency (you will need two)
The most common document that you can use to prove your residency are a computer-generated bill that reflects your name and address from a utility company, credit card, doctor, or hospital. The bill must be issued within 60 days of the date that you use the bill to prove your Indiana residency.
